We are requesting the following information for your foundation:
- IRS Form 1065 Schedule K-1 for years 2013, 2014, 2015 & 2016;
- IRS Form 8283 for years 2013, 2014, 2015 & 2016, if applicable;
- Financial reports, including as applicable, profit & loss statements for 2013, 2014, 2015 & 2016;
- Any recent appraisals or valuations of the assets initially contributed; and
- Information regarding the management of the assets by any other entity or office (if any).
Please also confirm for us the direct contact information for your foundation with a document that includes:
- Postal mailing address;
- Primary telephone number, indicating whether landline or mobile;
- Email address; and
- Your preference for form of contact.
If any one of the documents is unavailable for the requested year, please provide us with a written explanation of the document’s absence by CONTACTING US.
